JOB SUMMARY

F unctions as an expert clinician in oncology health care with a focus on Breast cancer and Palliative care services. The Breast cancer navigator assist patient through the continuum of cancer care from the time of diagnoses to survivorship. Functions as a clinical expert, consultant, educator and leader to plan patient care and ensure optimal patient care outcomes. Projects an image of professionalism in communication, appearance and conduct. Supports the mission of the department and organization. Participates in the continuous quality improvement process.

Breast Cancer Navigator Role: Collaborates with all members of the interdisciplinary team to:
• Establish relationship with newly diagnosed patients and guides them through their disease process
• Offers education and supportive services
