Good Turn Services is looking for Care Coordinator for our Home Care Services division at Karratha, WA.

ROLE DESCRIPTION

The Care Coordinator will be responsible for coordinating and managing care services for the participants. The Care Coordinator will also be responsible for rostering support workers, conducting assessments, creating care plans, and communicating with participants and their families.

REQUIREMENTS
• Care Coordination and Case Management skills
• Strong communication and customer service skills
• Experience in Home Care or Aged Care industry preferred
• Ability to work independently and as part of a team
• Qualification in Disability Care/ Aged Care/ Social Work preferred
